Pet Questionnaire

Prior to agreeing to care for your pet(s) I will require a number of details about them. By completing this questionnaire you should provide me with all of the information necessary for me to provide the highest level of care for your pet(s)
Some of the information I will require will include:

  • Your name

  • Pets name

  • Your address

  • Your contact number and alternative contacts

  • How I will be able to access your property

  • Whether or not I will be receiving a key (for repeat visits)

  • Where I should leave your key upon finishing the walk

  • Your pets immunisations: list required immunisations

  • Your pets medication

  • Any injuries/ disabilities

  • Your dogs veterinarian

  • What to do in the event of an emergency

  • Their likes and dislikes in regards to walks and interactions with other animals

  • Are they allowed treats

  • Are you happy with your dog being transported in my vehicle to pick up other dogs or to travel to a walk location
